This boy has fifth disease. Fifth disease is best known as a rash with a strong red flush on the cheeks, which looks like someone slapped them. This rash is a typical finding in this illness, also called slapped cheek disease. Notice the reddening of the cheeks. Despite the obvious rash, the child is alert, interactive, and otherwise healthy.
